Correction of Issued Birth / Death / Cremation Certificate

Correction of Issued Birth / Death / Cremation Certificate:
- Purchase Application form for Correction of Birth / Death / Cremation certificate from Birth and Death department of this municipality against Rs. 10/-.
- Submit the filled application form to Birth and Death department in this connection please note that "Change of name is not allowed however the spelling can be altered".
- In case of spelling mistake of the Parents name, first the correction must be done from the hospital / nursing home where Birth / Death occured then only applicant may apply.
- Applicant must attach the following documents along with the Application Form –
- Photocopy of Birth / Death Certificate issued by the Doctor.
- Photocopy of Birth / Death Certificate issued by the Hospital / Nursing Home.
- All Original Birth / Death / Cremation Certificates that were issued earlier must be submitted.
- Submit the requisite fee as per the Rate Chart into cash counter of Barrackpore Municipality.
- Corrected Birth / Death Certificate will be issued within 15 to 20 working days.
**Note: Applicant must have a legal relation with the child (Incase of Birth) / deceased person (In case of Death / Cremation).
