- Death Registration Process
Issue of Death Registration certificate:
- Purchase Death Registration Application form from Birth and Death department of this municipality against Rs. 10/-.
- Submit the filled application form to Birth and Death department in this connection please note that "Place of Death must be within Barrackpore jurisdiction".
- Applicant must attach the following documents along with the Application Form –
- Photocopy of Death Certificate issued by the Doctor.
- Photocopy of Death Certificate issued by the Hospital / Nursing Home.
- Photocopy of Burning Ghat / Morgue / Cemetery receipt.
- Photocopy of Receipt of submission of Voter Card.
- Photocopy of Receipt of submission of Ration Card.
- Cremation Certificate (If Applicable).
- Submit the requisite fee as per the Rate Chart into cash counter of Barrackpore Municipality.
- Death Certificate will be issued within 4 – 5 working days.
**Note: Applicant must have a legal relation with the deceased person.
