Issue of Birth Registration Certificate:
- Purchase Birth Registration Application form from Birth and Death department of this municipality against Rs. 10/-.
- Submit the filled application form to Birth and Death department in this connection please note that "Place of Birth must be within Barrackpore jurisdiction".
- Applicant must attach the following documents along with the Application Form –
- Photocopy of Birth Certificate issued by the Doctor.
- Photocopy of Birth Certificate issued by the Hospital / Nursing Home.
- Submit the requisite fee as per the Rate Chart into cash counter of Barrackpore Municipality.
- Birth Certificate will be issued within 15 – 20 working days.
**Note: Applicant must have a legal relation with the child
